Health & Wellness In Indian Country

WEAVE-NW is a program of the Northwest Tribal Epidemiology Center, funded through the CDC’s Good Health and Wellness in Indian Country (GHWIC) initiative. The overall objective is to strengthen and broaden the reach and impact of chronic disease prevention programs that improve the health of Northwest Tribal members and communities.

WEAVE-NW centers on developing community-inclusive, culturally relevant, team-based care to support chronic disease prevention. Creating programs—like traditional gardens or sustainable food systems—that encourage and provide prevention of chronic diseases like diabetes, heart disease, tobacco prevention, breastfeeding, and oral health through First Foods is an essential prevention strategy. Through this project, wellness programs emphasize cultural and social connectedness, along with physical activity, to improve community outcomes, including overall health, resilience, and prosperity.
