Emergency Incident Assistance Request Form
This form is meant to be used by Northwest Tribes that have experienced recent or current emergency related incidents that may require assistance. The information submitted is confidential and will be reviewed by the Executive Director of the Northwest Portland Area Indian Health Board for an appropriate response.
